ISAN is continuing to be actively engaging in chronic disease management on a number of fronts. A series of presentations on chronic disease management have been made throughout the London hospitals to introduce the chronic disease management model of care and how the London hospitals might contribute to the growing interest in CDM. Through the leadership of Chris O’Callaghan (Manager, Regional Stroke Strategy) information analysts at LHSC and SJHC, London have been brought together to identify the current role of the two hospitals in CDM. ISAN has also been exploring opportunities to initiate a CDM demonstration project.
The model being promoted is one that in various forms is being used on a worldwide basis.

Called the Care Model or Chronic Care Model, this model takes a comprehensive and longitudinal look at the on-going needs of people with chronic diseases and talks about how an integrated, cohesive and proactive approach, both within and outside the formal health system, can improve health status as well as functional and clinical outcomes.
In Ontario, the model being developed spans both health promotion and health care and is called the Ontario Chronic Disease Prevention and Management (CDPM) Framework. While not formally released, the following diagram describes the component parts of the system and how they are connected.
