Continuity of Care Committees

The Continuity of Care Committees (Adult and Pediatric) are multi agency partnerships between the following agencies: CCAC of London and Middlesex, Integrated Strategic Alliances and Networks, London Hospitals, Long Term Care Facilities, Community Health Service Providers, London Regional Cancer Centre, Children's Hospital of Western Ontario, Child Parent Resource Institute, Ministry of Health and Long-Term Care, Thames Valley Children's Centre. These partnerships focus on Adult and Pediatric health related issues in London and Middlesex. The committees are accountable to the organizations that are represented in the partnerships, but the committee as a whole does not presently have a defined body to which it is accountable.

The Adult Continuity of Care Committee has the following responsibilities:

  • To identify and collaboratively address issues and problems associated with the provision and coordination of care to patients/clients who are transferred between hospital and community-based services.
  • To proactively identify and develop innovate ways to improve the coordination of care to patients/clients transferred between hospital and community-based services.
  • To promote a shared understanding of health care delivery issues and new developments in the delivery of care as they apply to both hospital and community-based services.

The Pediatric Continuity of Care Committee has the following responsibilities:
Under this Agreement, the Partners agree to work together in the following areas of activity:

  • To identify and collaboratively address issues and problems associated with the provision and coordination of care to patients/clients who are transferred between hospital and community-based pediatric services.
  • To proactively identify and develop innovate ways to improve the coordination of care to patients/clients transferred between hospital and community-based pediatric services.
  • To promote a shared understanding of health care delivery issues and new developments in the delivery of care as they apply to both hospital and community-based pediatric services.
