For Health Professionals: How to Make a Referral

Inpatients

Requests for assessment and management are made by physicians/delegates through order entry in the Electronic Patient Record.

Outpatients

Adults (18 years and older) can be referred for assessment of acquired communication, swallowing, or voice disorders by family physicians within London-Middlesex, or physicians with admitting priviliges to London Health Sciences Centre. Patients eligible for service at St. Joseph's Health Care London, or other community resources, may be directed to seek service at those agencies.
