Discharge Planning
At the earliest opportunity, you and your health care team will begin planning for your discharge. Most patients return home when they are discharged.
- Please inform your health care team of any special needs you may have when you go home. (e.g. Community Care Access Centre, travel grant, previous services, visiting nurse, physiotherapy, home oxygen, etc.)
- Any prescriptions and follow up appointments will be given on the day of your discharge.
- Please arrange for transportation home once the doctor has given you a discharge date. You must notify the nursing staff if you are unable to make arrangements.
If you need more care, your health care team will work together with you and community partners to plan for an appropriate discharge. This will ensure that your health care needs can be met when you leave the hospital.
Time to Leave
- You will be notified of your anticipated time of discharge. If you are unable to arrange transportation, you may be asked to wait in a waiting room.
- Be sure to take all personal belongings including any medications or valuables, which may have been secured in the Business Office, Room D3-408.
If discharge is planned for a Saturday or Sunday, arrange to obtain your valuables from the Business Office through nursing staff on your floor.
Lost and Found
- Each patient is responsible for ensuring that all belongings are collected before leaving the hospital. If you have forgotten something, immediately call the Unit where you were a patient.
- Valuable items left behind will be held by Security Services for 90 days. Inquiries may be directed to Security Services: 519-685-8500 EXTENSION: 52281
Charge for Take-Home
- Supplies and devices not covered by OHIP such as canes, crutches, neck collars, embolism stockings, dressings, surgical supplies, etc. that are taken home with you should be paid for at the Business Office before you leave.
- If payment is not made within 15 days, an invoice will be sent and a $10 minimum amount billed. Note: Crutches are not returnable.

Prescription Centre
Before leaving Children’s Hospital, you may take your prescription to the retail pharmacy at Children’s Hospital. All major drug plans, credit cards and debit cards are accepted.

Hours: Monday-Friday 9:00 a.m. to 5:00 p.m.
