If You Have Questions, Comments or Concerns

If you are coming in for an appointment, we encourage you to write down your questions so you can ask your physician and care team at the time of your appointment.

If you are in the hospital, we encourage you to speak to any member of your care team. You can also speak to a Social Worker. Any member of your health care team can assist you in contacting the appropriate social worker at your request. You may also contact Social Work Services directly at: 519-663-3131.

Children’s Hospital will obtain family and patient feedback about areas of patient safety and family centred care through the distribution of question cards on the units. The hospital will use the feedback along with other evidence to target areas for improvement and measure the quality of care.

Comments, Compliments or Concerns About Your Care

We encourage individuals to ask questions, offer compliments, and voice their concerns during their visit or stay. However, we know not everyone takes this opportunity. Patient Relations Specialists at Children’s Hospital are available to assist patients and families with access to and knowledge of their rights and responsibilities. The Patient Relations Specialists will facilitate compliments, complaints and conflicts to a reasonable outcome as a mediator between the patient/family and the health care team. The specialists can be reached at 519 685-8500 ext. 55881 or 55882.
