A message regarding your discharge from London Health Sciences Centre

Dear Patient,


I am writing to welcome you to London Health Sciences Centre. Our goal is to do everything we can to make your stay comfortable, and to provide the care and treatment that is medically necessary.


Our focus is not only on your care and treatment while in hospital, but also your discharge from the hospital. The hospital is not the best environment for patients who no longer need acute hospital care. We start planning for your discharge as soon as possible. This helps ensure that we meet your needs on the day of your discharge, and that the appropriate services are in place, if needed, when you leave hospital.


We request your assistance with your discharge from hospital and ask that you or your family make arrangements for you to be picked-up by 11:00 a.m. on your scheduled discharge day. Although every effort is made to accurately predict the day of discharge, please note that you should be prepared for discharge to occur a day earlier or later than planned.


Most patients return home when they are discharged. However, if you need more care, your health care team will work together with you and our community partners to plan for an appropriate discharge.


LHSC cares for patients who require the special expertise of an acute care hospital. We are committed to working with patients and their families to improve health and provide compassionate, high quality care.


We want to ensure that we provide the right care in the right place and the right time. If you have any questions about discharge planning, please talk to your health care team. They will have a fact sheet for you with more information.
