Clinic Appointments: What to Bring

London Health Sciences Centre is a large, complex organization. You may wish to review the map of the hospital where your appointment is.

Be sure to give yourself a little extra time to get to your appointment if you are not familiar with the hospital or clinic location.

For your clinic visit, be sure to bring:

• Health Card

• List of medications you are taking.

• Review any instructions provided by the physician’s office (doctor’s name, clinic name, room/floor number, what to bring/do in preparation for the visit).

• Bring a friend/family members for support and if you need help getting to and from the hospital.

• Money for parking

• All of your questions, you may want to write them down to ensure they all get answered.
