Social Work Services

Social workers are professionally trained members of the health care team. Social workers provide emotional support and information services to patients and their families.

For example, social workers help patients and families adjust to the many challenges brought on by illness, hospitalization and treatment. They can help provide an understanding of the hospital system, the support available in the community and at home, and help connect you to those services.

Social Workers can also help patients and families to:

  • communicate with the medical teams,
  • make decisions about treatment and care,
  • help with your discharge planning.
