During your treatment, you may want some help at home. Your local Community Care Access Centre (CCAC) can help you get the services you need. South West CCAC Case Managers will work with you and your family to get the information and services you need. These services services are provided by highly qualified health care professionals with experience in oncology care and coordination. They will be with you throughout your cancer journey.
You do not need a referral from your health care team. You or a family member can call your local CCAC.
