Coordinated Intake for Ambulatory Mental Health Services

Coordinated Intake is the main point of entry for community referrals to access most of the adult outpatient services provided by London Health Sciences Centre (LHSC) and Ambulatory Service of the Mental Health Program at Parkwood Institute.

Eligibility Requirements

  • Serving individuals ages 18 to 64
  • Serving residents of London and Middlesex County
  • Available services are a match for individual’s needs

Exclusion Criteria

  • Needs that would be better met by other services

Referral Process


A physician or nurse practitioner referral is required using the Coordinated Intake referral form; a family doctor, nurse practitioner or a walk-in clinic may make the referral. Any available reports or information relevant to the referral i.e.) lab results, questionnaires are also appreciated.

Referrals are triaged according to urgency on receipt and the patient will be contacted by a Coordinated Intake nurse for a telephone intake. This phone discussion is necessary to determine the most indicated service/appointment.

Service Options


Current options available through Coordinated Intake at LHSC for acute care psychiatry include:

  • Psychiatry consultation
  • Treatment streams of the General Adult Ambulatory Mental Health Services including: The Cognitive Behavioural Therapy Program, The Emotional Regulation Program and Track to Wellness
  • The Urgent Consultation Service
  • The Traumatic Stress Service
  • The Early Intervention Psychosis Program (PEPP)                   *also accepting direct referrals and patient self-referral
  • The First Episode Mood & Anxiety Program (FEMAP)             *also accepting direct referrals and patient self-referral

Referrals for the Adult Eating Disorders Program are not managed by Coordinated Intake and should be made directly to this service.

Current service options available through Coordinated Intake at Parkwood Institute’s Mental Health Program for people with tertiary care needs include:

  • Transition Into Primary Care Psychiatry (only available for patients with a GP)
  • Ambulatory Services

Referral to the Dual Diagnosis Program and the Concurrent Disorders Program are not managed by Coordinated Intake and should be made directly to these services.

Service Limitations


Coordinated Intake is unable to provide linkage for individual, couple, marital or family counseling, ongoing care with an individual psychiatrist, or specific assessments for CAS, ODSP, court/legal purposes or to satisfy other third party requests.   Linkage to addiction services is not provided by Coordinated Intake; these may be accessed through Addiction Services of Thames Valley.

Crisis services are not provided by Coordinated Intake. Recommended options include:  London Mental Health Crisis Service (519-433-2023) by phone or in person at 648 Huron St..  For mental health emergencies there is a psychiatry team located in the Adult Emergency Department at Victoria Hospital (Commissioner’s and Wellington Roads).
