Coordinated Intake for Ambulatory Mental Health Services

Coordinated Intake is the single point of entry for all community referrals to access adult outpatient programs provided by London Health Sciences Centre (LHSC) and the Ambulatory Services attached to the Mood & Anxiety and Psychosis Program at Regional Mental Health Care–London (RMHC-L).

Eligibility Requirements


Eligibility requirements include:

  • being between the ages of 16 and 65
  • being a resident of London and Middlesex County
  • meeting the admission criteria of the service
  • having neither pending criminal charges nor active substance abuse.

Exclusion Criteria


Current/pending criminal charges are exclusion for group therapies.  Consultations for the purpose of a mental heath assessment are available on a limited base.

Referral Process


A physician referral is required; your family doctor or a walk-in clinic may make the referral. Your doctor is also encouraged to send any available reports or information relevant to the referral.  

Upon receipt of the referral, a Coordinated Intake clinician will contact you for a phone-screening interview. This phone discussion is necessary to determine the most appropriate available options for your care.

Service Options


Current service options available through Coordinated Intake include:

  • Various treatment streams of the General Adult Ambulatory Mental Health Services
  • Urgent Consultation Service
  • Early Intervention Psychosis Program (PEPP)
  • Traumatic Stress Service
  • First Episode Mood & Anxiety Program (FEMAP) at LHSC
  • Transition Into Primary Care Psychiatry (TIPP)
  • Ambulatory Services attached to the Mood & Anxiety Disorders
  • Psychosis Programs at Regional Mental Health-London.

Service Limitations


Coordinated Intake is unable to provide linkage for individual or marital counselling, ongoing psychiatrists’ care, and specific assessments for CAS, ODSP, court or forensic purposes.  Coordinated Intake also does not provide linkage for treatment of primary drug and alcohol addictions.

Crisis services are not available through Coordinated Intake. Recommended options include:  London Mental Health Crisis Service (519-433-2023) or the Adult Emergency Department at Victoria Hospital (Commissioner’s and Wellington Roads).

Your family doctor or walk-in clinic are able to access and/or contact Coordinated Intake on your behalf, by submitting a Coordinated Intake referral form by FAX to 519-667-6836 or by mail to Coordinated Intake Mental Health Care Program, London Health Sciences Centre, 800 Commissions Road East, LONDON N6A 5W9.

Phone requests for a faxed copy of Coordinated Intake referral form can be made to 519-667-6551.  Please note referrals are received between 8:30am and 4:00pm Monday through Friday (except statutory holidays).
