LHSC is required to maintain a 100% compliance rate for training on AODA, WHMIS, Fire Response and Evacuation, N95 (if required), Privacy & Confidentiality (P&C) Education Taken and Agreement Received and Hand Hygiene.

All employees, including physicians and laboratory scientists are required to complete the following programs at the required frequency:

TDG AND IATA TRAINING click http://www.lhsc.on.ca/priv/ohss/training/waste/index.htm#transport for recertification of TDG (every 3 years) and IATA (every 2 years). Please note, if you have taken the on-line training a wallet card will be mailed to you by OHSS so please fill out all fields of contact information accurately. If your require new certification please contact your Coordinator for more information.

Once all required training programs have been completed, fill out and submit the on-line StaffTrak Data Collection Form. A separate form must be completed for each training activity.
