Niastase®, eptacog alfa (activated) or recombinant activated Factor VII (rFVIIa) is administered at does of 35 - 90 ug/kg. The usual intial dose for hemorrhage is 80 ug/kg. It can be repeated if necessary q 2-6 hours until bleeding is controlled.
Like tPA, Niastase (rFVIIa) is a delicate protein. Reconstitute the vial with the appropriate volume of sterile water for injection.Slowly instill the dilutent into the vial and gently roll to mix. DO NOT SHAKE. This is the same method that should be used to mix other recombinant proteins including tPA (Alteplace), tenectaplase (TNKase, TNK) drotrecogin alfa activated [activated protein C] (Xigris)).
