Allison MacLean is a University of Toronto Masters of Nursing and Adult Gerontology Nurse Practitioner student finishing her second year of studies. She chose a clinical placement in Thoracic Surgery to help gain knowledge of the unique patient population and surgical care experience under the guidance of our Nurse Practitioner, Danielle Lozier, RN (EC), who was Allison's preceptor.
Allison identified an area of need within the patient population and developed a Quality Improvement Project focusing on patient hospital discharge and recidivism rates. She identified patients at risk of readmission and developed a post-discharge phone call program to help recognize potential complications in hope of lowering unplanned readmission rates.
We congratulate Allison on this Quality Improvement Propject and are excited that she will be presenting her project and findings at an upcoming Leadership Symposium Week in Toronto.
