Wound beds need to be assessed for presence of:
- granulation tissue (red)
- fibrin slough (yellow)
- eschar (black)
- other underlying structure
Some or all of these tissues and structures may be present in the wound at one time. Drawing a diagram of the wound bed that shows location and amount of tissue or structures will help assess healing processes.102
Ideally, a digital camera can be used to photograph the wound at intervals to document and assess the progress of the wound.
