All fields on the referral form must be completed. Incomplete referrals will be declined for service.
After the completed referral is received, the team will review the referral and determine eligibility for service. If the patient is eligible, the patient will be contacted by email or phone to attend our Mandatory Orientation Session, which is available monthly in-person and virtually.
The patient will be required to contact the program within 7 days of attending the Mandatory Orientation Session to indicate their interest in treatment. If we do not hear from the patient within 7 days, the patient will be removed from our waitlist and the referral will be closed. A new referral will be required, should the patient want to engage in treatment in the future.
If the patient contacts the program within 7 days, they will receive a date and time to complete a medical intake assessment with the nurse practitioner and a nutrition assessment with a dietitian. Following the assessment, treatment recommendations will be made and shared with the patient. A consultation note will be forwarded to the referring physician unless otherwise requested.
