FAMILY MEDICINE CLINICS

Program Description:


Ambulatory care is provided at our two off-site Family Medical Centres, the Byron Family Medical Centre and the Victoria Family Medical Centre. The Family Medical Centres provide a comprehensive, interdisciplinary approach to primary health care. The interdisciplinary team consists of family physicians, nurses, physiotherapists, social workers, and psychologists.


These centres are academic teaching centres affiliated with the University of Western Ontario and provide academic training opportunities for medical students, family physicians, nurses, nurse practitioners, physiotherapists, social workers and
psychologists.

  • Academic centres train Family Medicine residents and other health care personnel
  • Comprehensive seamless family centred care for patients
  • Nurses care for the children of patients they knew as children”

Byron Family Medical Centre (Commissioners Road W.)

  • Provide comprehensive care to approximately 8000 patients
  • Schedule 22,000 - 23,000 patient visits / year
  • Same day access available
  • Monthly patient seminars offered
  • Provide 24/7 physician on-call service and make house calls
  • Minor surgical procedures also done on site
  • Physicians and team members provide continuity of care by managing patient care at BFMC and on Family Medicine Unit when patients are hospitalized

Team Composition:


  • RNs, Physicians, Residents, Nurse Practitioners, Social Worker, Psychiatrist, Psychologist, Occupational Therapist, Pharmacist, Dietician, Secretaries, and Clerks

 

Victoria Family Medical Centre (Chesley Ave @ Hamilton Road)

  • Provide comprehensive care to approximately 6500 patients
  • Schedule 18,000 - 22,000 patient visits / year
  • Provide 24/7 physician on-call service and make house calls
  • Physicians and Social Worker provide continuity of care by managing patient care at VFMC and on Family Medicine Unit when patients are hospitalized
  • Procedures include sigmoidoscopy, circumcision, vasectomy and minor surgical
  • VFMC initiated first Annual Community Fair in 1994 which has now become the St. Julien Park Community Fair
  • Community Advisory Board ensures VFMC maintains strong community linkages (members include staff, patients, local community representatives)

    • RPN 3 full-time Receptionist 3 full-time
    • PT 1 part-time Secretary 2 full-time
