APPLICATION FORM

Minimum Requirements for Entry

1. Curriculum Vitae
2. Photocopy of medical degree
3. Evidence of successful completion of Specialty Training certification in Cardiology equivalent to the Royal College of Physicians and Surgeons of Canada
4. Letter of Intent
5. Three letters of reference
6. English proficiency either by TOEFL or interview
   

Upon receipt of the above, your eligibility for entry into our program will be evaluated.

Timeline

Decisions regarding fellowships are generally made by July 1st of the year before the fellowship starts (i.e. 1 year in advance). Applications after that point will be considered on an exceptional basis. Occasionally, a January 1st starting date can be arranged.

Ideally, applicants will contact the Program Director by email 18 months before the applied starting date (Dr. Lorne Gula, lgula@uwo.ca).

An interview is usually set up during one of the major scientific meetings (CCS, AHA or HRS) prior to the decision deadline. An interview in London is ideal when feasible to meet the entire EP team and see the facilities.
