To the communities we serve:
The update I have to share today is difficult. However, as I’ve shared before, it’s my responsibility as Supervisor to ensure that LHSC is addressing all concerns from our past so that we can focus squarely on what matters most – the great care our team provides to you and your loved ones.
Late last year, we announced that we engaged London Police Service (LPS) to investigate past financial practices at London Health Sciences Centre (LHSC) in response to concerns uncovered by our hospital. At the same time, we initiated a forensic audit by an external third party.
During the course of our investigation, we identified fraudulent activity within LHSC’s facilities management procurement practices that occurred between 2013 and 2024. We also identified that previous members of our executive team were alerted to these practices and failed to act against them.
As a result, we have now filed a statement of claim against Dipesh Patel (previous Vice President of Facilities), Derek Lall, Paresh Soni, Nilesh Modi, Varsha Patel, and several named companies in the amount of $50 million. Additionally, we have filed a statement of claim against Dr. Jackie Schleifer Taylor (previous President and CEO), Bradley Campbell (previous Corporate Hospital Administrative Executive), Abhi Mukherjee (previous Corporate Services Executive/CFO), and consultancy firm, Corpus Sanchez International Consultancy Inc., for breach of fiduciary duties. These three individuals are no longer with the organization and were paid according to their employment contracts and/or legal agreements. We are pursuing repayment in the collective amount of $10 million.
These statements of claim describe the findings of our investigation, including:
- Fraudulent behaviour in facilities procurement and construction contracting practices, including awarding of contracts with inflated pricing to companies owned and operated by LHSC’s then Vice President of Facilities Management.
- That this fraudulent activity was raised to Dr. Jackie Schleifer Taylor, Bradley Campbell, and Abhi Mukherjee who failed to take action and deliver on their fiduciary duties as executive leaders of the hospital by not informing LHSC’s internal auditor, external auditors, and Board of Directors.
We have posted the publicly filed statements of claim to our website:
- Statement of claim filed against Dipesh Patel, Derek Lall, Paresh Soni, Nilesh Modi, Varsha Patel, and several named companies
- Statement of claim filed against Dr. Jackie Schleifer Taylor, Bradley Campbell, Abhi Mukherjee, and Corpus Sanchez International Consultancy Inc.
As we vigorously pursue these civil cases and continue our forensic audit, we have provided these findings to London Police Service as they continue their investigation. At this time, any other staff involved in this misconduct are no longer employed at LHSC.
I recognize this is a deeply disappointing moment. It’s not who we are at LHSC and it’s not a reflection of our team. Since coming to LHSC a year ago, I have been privileged to meet and get to know many of our staff, credentialed professional staff, leaders, volunteers, Patient Partners, and learners. They are great people and provide great care, and I can tell you that each one of them is committed to our shared organizational value of accountability.
As we look ahead, we are doing the work to make sure this never happens again. This includes strengthening our oversight, governance, operations, policies, procedures, and financial practices through the work of our recent organizational review and executive restructuring.
Sincerely,
David Musyj
