Patient Health Information

Patient Records

A patient record (or health record) is a confidential file containing information about an individual's health history, including test results, diagnoses and treatment plans.

At London Health Sciences Centre (LHSC), patient records are managed by the Health Information Management department. They oversee the collection, storage and retention of your records and have a dedicated Release of Information office staffed with professionals available to help you access the information you need, when you need it.
