To be eligible to participate in the Complex Care for Kids Ontario (CCKO) program, the patient must:
- Be under 16 years old at the time of referral
- Be considered medically complex and technology dependent
- Live within our catchment area
- Have a primary care provider in the community
- Not currently be followed by another multi-disciplinary team (e.g. within a diabetes, cystic fibrosis or neuromuscular clinic)
Other eligibility criteria include fragility, chronicity and complexity.
