What to Bring to my Appointment

  1. Your child’s Health card.
  2. A list of developmental resources that your child has been referred to or is receiving with any supported documents (eg. Physiotherapists, occupational therapists, speech-language pathology, etc.)
  3. Completed ages and stages questionnaires that you received via mail or email.  If your child was born <29 weeks, at 2 years of age you will receive an email from q-global to fill out a questionnaire for a Bayley Scale for Infant Development 4th edition. A Child Developmental Inventory (CDI) at the age of 3 years.
  4. List of current medication and allergies
  5. Name and contact of pediatrician or family doctor your child is seeing
  6. If Children’s Aid is involved, the contact person’s information
