Please keep in mind the wait for an outpatient appointment could be up to one to two years depending on the epileptologist.
Please include the reason for referral, relevant history, EEG reports and medical imaging if available.
If requesting a referral to an inpatient Epilepsy Monitoring Unit, specify this is the reason for referral. It will still need to be faxed to one of the epileptologists listed above for approval.
