To receive a radiation monitoring badge (Dosimeter):
- Complete the Online Badge Request Form - Schulich Medical Students
- Once the form is complete, your request will be sent to the radiation safety team at LHSC and your badge will be issued for the date(s) requested.
- If you do not have a radiation dosimeter available, you will be required to step out of the room when x-rays are present.
Radiation Safety will issue the dosimeter to the appropriate badge board in advance of the program or rotation that requires it.
Note:
- If you do not have a SIN number or if you are pregnant and require a Fetal Dosimeter please contact Radiation Safety LHSC.
- Dosimeters are not to leave the hospital. Please leave on the OR rack.
