LHSC’s OWN Clinic is a service provider of the Workplace Safety and Insurance Board (WSIB) and cannot initiate new WSIB claims.
If you are an injured worker:
Please have your primary care provider or specialist submit the required forms regarding your workplace injury or illness to the WSIB to initiate a claim. If you have a more severe injury, or are struggling with recovery, once you have been assigned a WSIB case management team, you can ask about a possible referral to our program.
If you are a health-care provider:
Please submit your patient’s work-related injury or illness, using the required forms, to the WSIB to initiate the claim. The WSIB will then refer the patient to the appropriate service provider for treatment. If the patient’s injury may require a surgical or specialist consult in your opinion, or is struggling with their recovery, please either contact the WSIB, or make this note on existing WSIB forms (program of Care, Health Professions reports, or form 8s).
