Patients must be referred to the HEAL program by a health-care provider (physician or nurse practitioner).
Referral criteria
- Patient is under 18 years old
- Concerns related to living with overweight or obesity
Submitting a referral
Referrals can be faxed to 519-685-8098 or emailed to HEALprogram@lhsc.on.ca. Please include the patient’s detailed medical history, most recent blood work results and growth parameters or chart.
Referral review and patient notification
Once the referral is received, the HEAL team will assess it to determine the eligibility of the patient. If additional information is required, the referring care provider will be contacted.
When the patient is accepted, they will be placed on our wait list. Both the patient and the referring care provider will be contacted with appointment details.
Please note: Wait times may vary based on program capacity. Our average current wait time from referral to initial consultation is about two years.
