Hospital Employees
Hospital Employees will submit their COVID-19 Vaccine Certificate to Occupational Health as part of their health review.
Private Hires
Private Hires will submit their COVID-19 Vaccine Certificate to Medical Affairs at medical.affairs@lhsc.on.ca.
