Hospital ID Badge

Only one badge is required for both LHSC sites (Victoria Hospital and University Hospital) and can be obtained from either location.

Credentialed Professional Staff

To obtain an ID badge, please visit one of the two GO2HR locations:
University Hospital - Room CLL-102
Victoria Hospital - Room E1-500

Human Resources will use the Medical Affairs Credentialed Professional Staff and Trainee directory to verify your status for ID badge issuance. If you will require door swipe access to secured locations, a signed ID badge request form is required. 

Residents and Clinical Fellows

**For July 1 Starts picking up their ID Badge at an LHSC Site (Victoria or University) **

Please review the following schedule to determine when you are asked pick up your Hospital ID Badge based on your last name.

Hospital ID Badge can be picked up at either site in the following rooms:
Victoria Hospital: Room E1-500
University Hospital: Room CLL-102

  • You may be asked for you hospital appointment letter in order to obtain your badge; please have it ready.
  • Residents and Clinical Fellows (not including Visiting Electives) - Please drop off your Life Insurance Enrollment Form to the representative when you get your ID Badge; this form must be handed in as a hard copy in person.
  • If you will require door swipe access to secured locations, a signed ID badge request form is required.
Medical Secretaries and Admins
  • Have your physician leader complete the ID Badge Request form on the Human Resources and Workforce Wellness website. If your role is changing and your current badge will require updated access, please email Medical Affairs at medical.affairs@lhsc.on.ca.
  • Please visit Human Resources and Workforce Wellness for information on where and when you can obtain your ID badge; you will need to bring government issued identification.
