Please reference this sheet after you are discharged from the hospital when you are still coming into the C7 day hospital for follow-up appointments (typically from day 14-28).
After you have been discharged to your hematologist, you will not be able to use the CAR-T telephone line.

If you call and there is no answer/reply, please wait 15 minutes and try again. If there is still no answer, please call the C7 Charge Nurse at 519-685-8500 extension 75031.
Reasons to call 911
- Difficulty breathing
- Bleeding that will not stop
- New seizure
- Chest pain
- Have fallen and can’t get up
- Loss of consciousness or can’t wake someone up
Reasons to immediately call your outpatient CAR-T team
- Fever of 38°C or higher (will require immediate assessment and admission)
- Chills or shakes
- Uncontrolled vomiting or diarrhea
- See blood in your vomit, pee, or poo
- Central line or tube issue
- Swelling in your hand, feet, face, neck, or abdomen
- New or worsening pain
- Nose bleeds or blood from your central line that is difficult to control
- Confusion
- Vision changes
- Any other urgent questions or problems
If you experience any other major changes, please report them to your healthcare team.
