Referral Management
LHSC’s Clinical Neurological Sciences (CNS) team is committed to a referral process that is both efficient and people-centred. Upon receiving a referral, the team first reviews the referral to ensure all necessary information has been provided. This includes, but is not limited to, patient demographics and referring provider details. Referrals without the required information may be returned for re-submission.
Most CNS referrals then undergo clinical triage to assess urgency, appropriateness, and the type of care required. This step ensures the referral is directed to the appropriate specialist or service and prioritized based on clinical need. Referring providers will be notified of the referral decision in a timely manner, in keeping with the expectations outlined in the CPSO’s Transitions in Care statements and OMA’s Referrals Checklist.
