LHSC’s Clinical Neurological Sciences Program uses a Central Intake model to provide referring providers and patients with a standardized and consistent referral process.
Send referrals using the fax number that corresponds to the referral type:
- Urgent Neurology Clinic referrals: 519-667-6766
- Urgent Transient Ischemic Attack (TIA) Clinic referrals: 519-663-3140
- All other CNS referrals - CNS Central Intake fax number: 519-663-3204
The CNS Central Intake fax number is for referral-related documents only. For outside reports or results, prescription renewals, form requests, and other non-referral documents, use the Clinical Directory to identify the appropriate office fax number.
Important Note About Referral Redirection
CNS Central Intake may redirect a referral to the most appropriate CNS provider, when applicable. If you do not want the referral redirected, indicate this clearly in the referral documentation. In keeping with CPSO expectations, CNS clinics and individual providers endeavour to respond to referral requests within 14 days of receipt.
