
Our multidisciplinary team is made up of Transition Navigators, Educational Liaisons, Child Life Specialists and Indigenous Wellness Consultants. Together, they nurture each patient along their personal growth and development journey.
Transition Navigators
Transition Navigators help patients identify and resolve potential barriers to care when graduating out of Children’s Hospital. They are responsible for patient advocacy and education, as well coordination of care.

Educational Liaisons
Educational Liaisons guide patients and families through educational transitions, such as moving from school to hospital and back to school again. They do this by educating patients on their rights to schooling and accommodations or other types of supports that can help them succeed in their studies.

Child Life Specialists
Child Life Specialists use developmentally appropriate methods to educate patients about their medical conditions and health-care experiences. They support social-emotional skill building and encourage goal setting that nurtures informed decision-making, self-advocacy, independence and medical compliance. They also guide families through changing dynamics to support readiness for transition.

Youth Indigenous Wellness Consultants
Youth Indigenous Wellness Consultants support mental health and cultural wellness for all Indigenous patients at Children’s Hospital. They do this by building meaningful therapeutic relationships with each family and fostering a sense of belonging and safety.

Manager
The Manager provides operational support to the team. They also advocate for resources identified by team members through patients and caregivers, as well as supports opportunities for partnerships in the community.
