The John H. Kreeft Headache Clinic provides specialized assessment and management of migraine and other headache disorders for patients over 18 years of age. Care focuses on establishing a diagnosis, optimizing acute and preventive treatment, and improving patients’ daily function and quality of life in collaboration with their primary care provider.
Referral Criteria
Referrals must be addressed to a specific provider: Dr. Paul Cooper, Dr. Tommy Chan, or Dr. Catherine Sukhavitskaya. General referrals to the Headache Clinic without a named provider will not be triaged.
Dr. Tommy Chan is currently not accepting new referrals due to capacity limitations. This may change in the future; please check this page for updates.
Referrals should include:
- Headache history, including onset, frequency, associated symptoms, and functional impact.
- Previous and current headache treatments, including doses, duration, response, and reasons for discontinuation.
- Frequency of acute medication use.
- Relevant examination findings, investigations, and previous specialist assessments.
Patients with sudden-onset severe headache or other features suggesting an urgent secondary cause should be assessed through the appropriate emergency or urgent neurology pathway.
