Help us to know our patients

Please talk to us about your family member’s wishes so we can ensure that we are providing care the way your family member would want. Your family member’s nurse will give you a document to complete called “Help us to Get to Know our Patients”. We ask that you take the document home and complete it with the help of other family members, so that you have time to consider your family member's values, beliefs and wishes.
