What to Bring and Expect

• A valid Health Card or proof of alternative health insurance e.g. UHIP
• Two pieces of identification with date of birth, photo identification is preferred
• Any medical information that you think may be of importance to the physician
• A list of questions - please come prepared
• The clinic is extremely busy, please plan on spending one to two hours for your appointment
• Be prepared to pay for parking by cash or credit card
• Tim Hortons is located on B2, B3 in the cafeteria and D2, with a 24/7 Tim Horton's on C3
• Please limit the number of people you bring with you to your appointment
• ATM machines are available on B1 and Level 2 across from Tim Hortons
