Oncology Patient Navigation Program

March 14, 2025

  • Step 1: Meeting criteria testing complete with suspicion of cancer.
  • Step 2: Referral to OPNP can be sent.
  • Step 3: Referral received and triaged. 
  • Step 4: Registered with OPNP and sent to next available MRP.
  • Step 5: Appropriate: 
    • Note added to chart and referral sent back to referring provider. OPNP will close the chart.
  • Step 6: OPNP will contact patient directly via telephone. 
  • Step 7: OPNP will navigate patient through staging as ordered by MRP.
  • Step 8: OPNP will become point of contact for patient until surgery or Cancer Centre.
  • Step 9: OPNP to close if: 
    • Surgery complete.
    • Patient is treated at Cancer Centre.
