Monitoring performance and evaluating progress are important because they help to ensure that services are effective, efficient and aligned to the needs of patients in our region.
Our focus in the current strategic plan is to:
- Coordinate care to improve patient flow and reduce wait times. This will help increase patient satisfaction and reduce hospital readmissions or emergency visits.
- Standardize systemic treatment pathways, education and palliative care to promote consistency in care delivery, and enhance patient knowledge, symptom management and quality of life throughout the cancer journey.
- Improve the effectiveness of the South West regions’ cancer system through better patient experience and outcomes. We will leverage technology to refine system processes. We will enhance prevention initiatives such as smoking cessation and increase screening rates for breast, colorectal, cervical and lung cancer. Earlier detection often means less treatment and better prognoses.
- Engage patients and providers to remove barriers and increase equity in access and use of services. Success will be measured through improve provider morale and reduced burnout, enhanced patient-reported experience scores, reduction in missed appointments, and better adherence to treatment plans.
Through monitoring and evaluating these core enablers, we ensure our efforts are aligned with patient and provider goals for continuous improvement and better outcomes.
