A patient portal is a secure online tool that gives you convenient access to your personal health information. They present a simple way to stay informed and engaged in your care by giving you access to test results, reports, and other important records.
Patients can access their health information from LHSC at any time, from anywhere, through three secure online patient portals. These portals may overlap and complement each other in some cases; depending on where you receive care, you may be eligible to use more than one.

MyCarePortal
MyCarePortal provides patients with a free, secure way to access their personal health information documented in OneChart.
Patient enrollment at LHSC will begin in spring 2026. As of September 23, 2025, patients who enroll at the following participating regional hospitals and also receive care at LHSC will be able to view their LHSC record through the portal:
- Alexandra Hospital (Ingersoll)
- Listowel Memorial Hospital
- South Huron Hospital Association
- Tillsonburg District Memorial Hospital
- Wingham and District Hospital
Once enrolled, patients will be able to view diagnostic imaging, pathology reports, laboratory and microbiology results, and discharge summaries finalized on or after October 1, 2018.

ConnectMyHealth
ConnectMyHealth is Southwest Ontario’s patient portal that provides access to your health records from participating hospitals in the region. Once registered, you can:
- View various types of health records, including radiology reports and images, laboratory and microbiology results, hospital discharge summaries, and more!
- Share your records with other ConnectMyHealth users such as your family or caregivers.
- Subscribe to be notified by email when you have new results available to view.
- See your upcoming hospital appointments.
ConnectMyHealth is safe and secure, and can be used on desktop and laptop computers or mobile devices. ConnectMyHealth does not require a separate app, and there is no cost to use it.

PocketHealth
PocketHealth offers London Health Sciences Centre (LHSC) patients and their families access to their medical images online.
Once patients enroll for access, they can view all of their medical images that are stored in LHSC’s system, share imaging instantly with others of their choice, or even burn their own CD/USB from home. There are two fee options available that are payable to PocketHealth beginning at the time of enrolment which includes permanent access to all previous imaging as well as any new exams: Flex ($10+HST/month, cancel anytime) and Unlimited ($49+HST/year).
